My Fiancee family moved from Cagayan de Oro after she graduate from High School at 17 to Pureto Princessa.

I was on the Manila Embassy Site it states "A fiancé(e) is considered an intending immigrant and therefore must present documents similar to those required for an immigrant visa applicant. These include: a valid Philippine passport; a copy of the applicant's birth certificate issued by the National Statistics Office (NSO) and printed on NSO security paper; legal documents proving the termination of a previous marriage; NBI clearance; police certificates from all foreign countries where the applicant lived for at least six months starting at the age of 16".

Does she need to go that far back to age of 16 and get a Police Certificate from Cagayan de Oro?

If so, how could she get one without going back to Cagayan de Oro?

Not necessary. Police certificate is only required for applicants who lived outside the Philippines for 6 months. All she needs is the NBI clearance/cert..

I think the NBI clearance will cover her but I'm not 100 percent sure.
