The topic really says it all. Not sure what name I should use. She is changing her name to mine so I'm not sure if all the AOS forms should be filled out with her new last name or her maiden name. Thanks

If she want her greencard to be in her married name then fill the form in with that name. If she uses her maiden name on the forms and then wants to change it to her married name later it will cost $$$ for a new card.

Use her married name. It will be on the GC. And on some applications there is a line "Other names used" - that is where you put her maiden name.
