So, we FINALLY FOUND a doctor in our area that is able to transcribe the vaccination from Korean hospital onto the I693 FORM. Without any questions or hassels of asking us to complete the entire physical. Most medical civil surgeons are requesting that and charging a lot of MONEY....

there is one in Santa monica , think it's on Washington blvd, dr ben something charged me $35 for just transcribing I-693 I got my vaccinations at my normal doctor. Payed upfront picked it up next day. She just made sure that I had the full medical already and understood that if I was missing it I could denied she( receptionist) didn't make a fuss about medical at all.
