I have called a number of different places about transferring the info onto the vaccine supplement. They all want to make it an office visit/physical, and charge $100+. I thought we should just be able to drop it off for them to copy, and that it would maybe cost $20. How much have you guys paid to have your shot record transferred? Were you able to do it outside of an office visit?

Do you work for free? Some doctors fill it out for free as a courtesy, but don't expect all to do this.

Check other designated CS office you might be able to find one cheaper than that.

We paid $35.00 to have the information transferred from the Vaccination Worksheet onto the Vaccination Supplement.

We just faxed in the Worksheet to the doctor's office, they transferred the information, and we picked it up at the doctor's office.

We called around to a lot of offices in our area, and found the prices to be very different and ranged from $35.00 to $200.00. So, call around and try to find one that will do it for a reasonable price.

I called around in the Melbourne Florida area and the cheapest I could find was $75 which I think is rediculous to fill in a piece of paper but I don't have much of a choice. You might just have to pay whatever they want if you can't find anything cheaper.

We paid $35 for what they called an office visit charge.

We paid $40 for ours and the doctor filled in the worksheet while we waited in the waiting room. Took us about 20 minutes in total and we were not charged for an office visit.

I paid $50 and had to go into the office, wait while the paperwork was done by the receptionist, then checked and signed by the doctor> It took about half an hour. There may have been cheaper but this was the closest and easiest to get to so I saved on gas, time and frustration. I was going to take it to my AOS interview since I only included the worksheet in with the package, then was approved without an interview, so I still have the envelope here as an expensive souvenir.

I know its a little expensive but lets face it this people work to get to where they are right now. Lets not be too negative . We paid $25. You don't have to do the complete physical again all they have to do is transfer the information. Good luck.
