My fiancee recently picked up his packets but made mistake with filling the forms. Instead of filling the forms with his old address(the one i used for everything) he used his new address. He just moved from Accra to Kumasi. My problem is that in case we are put in AP(God for bid) and they do field investigation at Kumasi, no one really know me there so it might be difficult to get the information they need and might affect us. Does any one know if you can ask for another forms? I'm trying to aviod the whole address change. It could slow down things as well. Any help will be appreciated.

She meant the forms in the packets when you pick them up from the embassy. DS 156, DS 156k, DS 230 etc.

I downloaded forms for my fiance on the Ghana Embassy website. Filled them out for him, printed them, then scanned them into a PDF and emailed the complete forms to him for his signature. I had to print and scan them because you cannot save them to your computer. It worked fine.
