Where can I find a I-693A? I found I-693, but can't find anywhere to download just the vaccine supplement.

Also, with the I-864 tax records- is just the W2s enough, or do they need a copy of the entire return?

We tried looking for it too and nothing. The doctor we went to had it.....looked like it was xeroxed about a million times. Whoever you go to, to get the supplement will have the form.

Here it is... The CDC has it too. It's not actually called the I-693A. It's.....

The doctor gives the applicant the vax supplement form, completed.

The I-864 is *very specific* about what must be included; I recommend that you re-read the instructions.

PS: tax transcripts are a free, fast and fuss-free way to report your tax return.

You need to send the complete tax returns, 1040 with W-2 if you are filing as an individual.
