After calling the toll free number, I could NOT get a live person on the phone.

I even called directory assistance, and when I asked for the number for "Service Ontario"........."We have a listing for Purvis. Would you like the number?"........."NO!!"

I challenge anyone here who is from Ontario, to find me a direct line to talk to someone at the Service Ontario office in Pembroke. The toll free number is 1-800-461-2156. Have fun. :P

My next option is to go down to the office again.

Hello Erin,

Unfortunately Pembroke doesn't have a Service Canada office. The Office of the Registrar General (they take care of Birth, Marriage & Death certificates) is in Thunder Bay and the best way to contact them live is on their Toronto based number 416-325-8305 you will be able to speak with someone live that can answer all your questions and provide you with possible alternatives.

Hello Erin,

Thankyou for the Thunder Bay number. I was referring to the Service Ontario office, and yes there's one in Pembroke. I got my application form there. Since I don't have a car, and it's a distance to go, I emailed Service Ontario using their "contact us" link on their webpage. I told them nobody at my bank would be a guarantor, I asked if a guarantor is necessary when applying for the long form, and asked if Pembroke had a direct number I could call. I got this reply:

Dear Sir/Madam,

Thank you for your email. In the fall of 2001, the Ontario government passed a new law to change and strengthen the security and safeguards in place for issuing birth certificates and to protect Ontarians from identity theft and fraud.

The role of the guarantor is to certify that information provided on a birth certificate application is as complete and accurate as possible. If required, a qualified guarantor must also be available to verify the information with the Thunder Bay Production and Verification Services Branch.

We regret that this has caused you any inconvenience. We are sure you can appreciate why these changes have been made. Your Ontario birth certificate is the key to your identity. The rules are more stringent and more secure.

The phone number for the Service Ontario office in Pembroke, Ontario for general government information is:

1 800 268-8758 Toll free in Ontario.

They also gave me the number for Thunder Bay. So I'll be making some phone calls tomorrow morning, or perhaps bike down to the office in Pembroke.

I suppose I could go down to the walkin clinic, since I don't have a family doctor, and ask one of the doctors there if they'll allow me to use their name. But it's not like they know me personally either. Geeeeeeez!

I think calling Thunder Bay would be the better option as Service Canada does not have your original birth record they just provide information on how to obtain the information.

Good luck

I went down to the walkin clinic the other day and they had a lineup. So I left my application with the receptionist so she could ask one of the doctors there if I could use his name as guarantor. Went back and hour later..."No problem.

Dr. O said you could use his name."
