Applying for Canadian long for birth certificat

1. Name and address of doctor attending the birth...I put "unknown"...(I'm pretty sure that's ok since my mom has no idea what his name was) I also put birth weight as "unknown." I'm assuming since I'm not applying for a certificate for a newborn, they've probably got all that info about me on file anyway.

2. "Why are you requesting this information?"...I put "Immigration purposes. K-1 Visa"...is that enough? Also in the same section it says I "MUST check one of the following boxes"...I checked "First time applying for Birth Certificate/Certified Copy of Birth registration."

3. For the guarantor, it says that one person I could use is a signing officer at my bank. Has anyone had an experience with this type of situation?

And a hypothetical question...

I already have my short for birth certificate (the regular credit card sized one). Is there any chance that this will be enough? I was told it would be 6-8 weeks before I would receive the long form. Just seems like such a longgggg time. *sigh*

1. My mom thankfully knew this. I don't think it's a big deal that you don't know. They just ask the info so they can find your certificate faster.

2. Hm, I don't remember answering that one, but I assume what you put would be good enough. I think even putting "Visa related purposes" would be good enough. I did check the "First time applying for Birth Certificate/Certified Copy" box, though.

3. I used my family doctor, so I suggest using yours if you have one.

And no, I don't think the short for one is acceptable. For some reason, my fiance's lawyers (I wish he didn't hire them, even if they were cheap) said my short form wasn't good enough, so I had to apply for the long form. And you'll need the long form for the interview anyway, so it's best to get it now.

And the wait time lies. I got mine in less than a week. It all depends on how much info you can give them so they can find it faster.

The first question is used to verify the information on your birth certificate. Not sure if saying unknown will be an issue, probably not - but if you know your birth weight, you should include it. We didn't know my Dr.'s address - but we had the name and it was no trouble.

They say it takes 6-8 weeks, but I got mine in 5 days or something like that, it was fast.

The long form birth certificate must show the names of both your parents, so the credit card sized one will not be sufficient.

I ordered mine online in B.C. and got it within a few days.

Ok, the long form it is. I'm not taking chances. Besides, 6-8 weeks did seem quite a long time to be waiting. My mom applied to get a new birth certificate last year because hers was old and in bad shape, and I think she got hers within 3 weeks.

The reason I didn't order mine online is because I had questions about it before sending it in, so I wanted a hard copy to refer to.

I was told at Service Ontario, that the guarantor doesn't need to SIGN the application. I just need to enter their name, where they live, their occupation and a phone number where they can be reached. They only need to know that their name is on it, and to maybe expect a call.

I WOULD have used my family doctor, but I don't have one. You know how it is these days...you move from one community to another and have to get on a waiting list for a doc. I just use the walkin clinic, and I don't have the same doctor each time.

So hopefully someone at the bank can do it for me, since I've been banking there for almost 4 years now.

Thanks for all your replies.

"Ontario: Applicants should obtain "large" certificates or certified copies of a registration from the Office of the Registrar General in Toronto (Macdonald Block, 900 Bay Street, second floor, tel: 416-325-8305) or at Ontario Land Registry Offices and Government Information Centers located throughout the province. Further information, including locations and information on ordering by mail is available online.

Note: For marriages in Ontario, the certificate torn off the marriage license and given to the couple at the conclusion of the ceremony confirms that the marriage took place, but is not a legal document. Applicants must obtain a marriage certificate or certified copy of marriage record from the Office of the Registrar General." Source: http://travel.state.gov/visa/frvi/reciproc...ocity_3531.html

(I'm in Quebec, and I received mine maybe 10 days after ordering it online - I guess 6-8 weeks is a worst case delay)

Good luck :)

long form is needed for AOS and -depending on the state you move to- aparently a lot of them request long form birth cert for proof in person to pick up the marriage certificates and licenses.

You can apply for the long form on line, pay a little more ($100) and get it within 72 hours...there are a few third party companies that offer this service, you can just google it....

Long form is good to have either way, there will be other times down the road when you will require it...

I had to order my long form BC several years ago, and I'm amazed that you'd need all that information to get it! I'm not doubting you, but I'm sure I needed nothing like they're requesting from you. After all, this is YOUR BC! And a Guarantor? Wow, amazing. Hope it comes sooner than expected!

The whole Guarantor thing is stupid in my opinion.

About 19 years ago when my mom got my brother's, she didn't need one.

Maybe there's been some fraud or something, who knows.

see here $35 for long birth certs, took about a week to receive. Also can get marriage certs.

I have the same form on hand that is available online.

I was at my bank yesterday and talked to 4 people (starting at the teller, all the way up to manager) to see if someone could put their name down as guarantor. Nobody would do it, regardless of the fact I have been banking at the same bank for 3 years now. They read on the form that they must know me for at least 2 years. They kept saying they didn't know me PERSONALLY. I pointed out that the word "personally" was not mentioned. They said they couldn't sign it, and I pointed out that there was no place on the form for a guarantor's signature, only their name, occupation and phone number. I explained to them that the gov't only needed to verify that I did indeed have an account there and that I WAS an actual person. LOL Nope...nobody would put their name on it. :bonk:

So.........I'll be on the phone to Service Ontario this morning..........blahhhhhhhhhhhhh....."Push 1 for English........."
