Vaccination supplement form for I-485

Here's our predicament:

We are ready to file the AOS, but Andrea and I just realized we are to include a vaccination supplement report. When she got her medical in late May, the doctor never gave her a copy of the report he made. So at the POE, she submitted the sealed envelope and that was that. Currently, Andrea has a vaccination record book that is certified by the World Health Organisation. Can she make a copy of this book (recognized by pretty much every doctor) as proof of her vaccination supplement? If not, what do we do?

No. But you can take it to a civil surgeon and have him transfer the information to the I-693. The DS-3025 which is given at most medical exams if you bring this vaccination record with you, is also acceptable for K visas with a medical exam within one year. Did she not take her vaccination records to the interview?

The first question is, did she submit any of her records at the medical exam? K-1 visas are not required to submit to the vaccinations during the medical, which means that you have to do it here in the states if you didn't there. They can just do the Medical and it's up to you later to either show you've met all of the required vaccinations or that you get them then have the I-693 filled out. You have to take her vaccination record that you have and have a Civil Surgeon fill out the Vaccination portion of the I-693. They will tell you if her records show she already has all the required shots or not. We finished ours just last month. It was difficult to find a doctor that understands the K Visa process. VJ was the ultimate source of all our information. We eventually did what we learned from others here which was to send all of her vaccination documents to a Dr. L. E. Arnold in Austin, Texas. She is pregnant and so between that and the records she already had, she didn't need any. At the medical exam, they were supposed to give you a COPY of the DS-3025, Vaccination Documentation Worksheet, whether or not it was finished. My wife got hers, but it only showed she had a VH, Virus History of Chicken Pox. So that was all we had on it anyway. If you call the doc in Austin, they will tell you if they need the DS-3025 or not. They told us to send all we did have and they will sort it out. I suspect it's not really needed if she refused the vaccinations as my wife did at the medical exam, because they know it's ok to have it done here at your further expense. I read on the I-485 that you are supposed to submit a copy of the DS-3025, Vaccination Documentation Worksheet if that's what was given to you when you took the vaccines at the exam, or if not, then there is a new form called the I-693 that a Civil Surgeon would need to fill out here, then he gives you a sealed copy for the I-485 and photo copies for your records. I suggest you compare her shot record with the CDC's vaccine requirements so you will know if she has any she may need in addition to those she already has. Google CDC and keep searching their website for Adult Vaccines required, those are what she will need depending on her age and other factors. Good luck, this was our biggest difficulty in the AOS process also.
