My guess is that I just bring my vaccination info and chest X-Ray results to the civil surgeon in my area and get him/her to fill out the I-693? Thanks!

Yes bring the vaccination sheet from the medical in Canada, I do not think you will need the chest x Ray. I have never heard of a CS asking someone for it in order to transcribe.

Also, would I need to wait for my SSN to fill out the forms? Thanks!

No you do not need your SSN # in order to file for AOS.

Great, thanks! Also for the AP form, what do you put for the "Intended date of departure" if you don't know it? Thanks!

I think I'm putting "Unknown" and attaching a suppliment page.

It's found here: http://www.visajourney.com/forums/index.ph...p;page=examples

Under the AOS section. I'm just explaining that I want AP incase there's a family emergency or death in the family when I don't have my green card.

Unsure if they'll accept it, but may as well try. I dont want to have to scramble for emergency AP if that happens.

I'm not sure why we would need to send in the I-693 (is that what they mean by the vaccination supplement)? The medical that we got from Toronto is good for a year, so I assumed that we would use that. Thanks.

Your medical is still valid, so there is no need to redo it, but you will need to have a civil surgeon transcribe your vaccination records onto the form I-693. Some have been approved without it, but there have been several RFE's issued in the last two months or so. If you chose not to send it you will need the form if you end up with an interview, so its smart to get it done sooner rather than later.

I just made it up, or guessed. Use the example Rhiann posted, it worked out well for me

Because we sent off my AOS near the end of October, I had put a date before Christmas stating that I'd like to go home for that. But because of my RFE everything got put on hold for a while anyways. Guess I should have just put "unknown" and state family emergencies. ( I did that anyway)

Best wishes for a speedy AOS!

GAH, waiting for the RFE letter sucks. I just cant help but get panic attacks every so often about what it could possibly mean and if it means I have to leave the country, and when can I work blahblahblahblahblah.

USCIS says they mailed my RFE on Dec 31, but no mail yet :( Would anyone have an idea how long it takes?

Also, is there a customer service line I can call? I tried to call the 1-800-375-5283 number, but it wont let me get to a customer service rep.

UGH this is even worse than waiting for the K1 interview :P

My RFE took over a week to arrive. Maybe it was just because of the Christmas rush, but it seemed like forever!

Now that I have a second or two...I thought I'd post an update on the whole AOS/RFE experience to date.

The RFE we received in early December was indeed for the lack of an I693. They did not accept the photocopy vaccination sheet provided by Woking Medical in Vancouver. A round of calls to civil surgeons in my area found several who wanted to overcharge for the whole exam, and one who was willing to just do the vaccination form for a $30 fee. One week later, and the form is back in the mail.

They 'receive' the returned forms on Dec 24th, but only officially 'touch' them on the 29th.

On Jan. 1st (at 7am!) I wake up to a txt message indicating a touch on all three forms. Touch one: GC is meaningless...nothing happened. Touch two: AP 'is approved' and a letter with instructions has been sent. Touch three: EAD 'is approved' and a production order for a card has been issued. Expect your card to be mailed in 30 days.

So overall, I'm pretty excited! Hopefully by the time February rolls around, I'll have the EAD card and can actually start working again. That will be nice, because things are getting tight with only one of us working right now.

Just waiting on the GC itself now. Last update was 'we'll let you know what's happening within 60 days.' I take that to mean it's going to be months longer before I actually have the card in-hand, given the additional time for interviews/Cali processing after approval. It's nice to know that things are moving along though!

Happy New Year everyone... may your AOS experience be short and sweet!

Quick question for all of you AOS'rs...

I swear I came across this answer at some point, but for the life of me can't find it now. When it comes to the alien registration number, my visa has A#[8digits], but my NOA2 (approval) has A0[8digits]. Which is correct? On the I-693 form, it has room for up to 10 numbers in that section, however the G-325A only has room for 8 digits. Am I correct in assuming I ignore the zero in front of the number on my NOA2?

Congrats on the GC/AP approval Michael+Tracie!

Sam- You're right, just ignore the zero. Everyone's A# has a random zero in front of it. Put it with or without, they will still know what you mean! I put it without the zero on all of my AOS forms.
