Vaccination Sheet

I'm gathering the papers for the AOS package and I will probably sent them during the 3rd week of August since we're have our civil wedding on the 21st of August.

When i had my medical in March, All of my vaccins were good but i needed the gardisil. With the do not open envelop for the embassy, there was a photocopy of my medical vaccinations. I don't know how this form is called. It has been updated with the 2nd shot of gardisil and I am still missing the 3rd shot of Gardisil, it is due mid-september. So when i file for the AOS, i wont have my 3rd shot of gardisil yet.

Do i need to send a copy of this sheet with the AOS?

Or do i need to go see a civil surgeon to do the i-693?

Or what is the thing with this Dr. Arnold in TX?

I'm kind of lost in which paper i have to put in the AOS file...

I think you really need to see a civil surgeon to sign for your form i-693 because you need to include that form in your AOS. That's what I did in my case and it worked

Technically you need the Vaccination form filled out by the CS. However, USCIS may except your DS3025 so its really up to you. If your not in a hurry, I'd skip it and see if you get an RFE.

With regards to HPV, since you already have shot 1 you are good to go for AOS.

We are kind of a hurry so I think I will go see a CS. I remember seeing the list of where i could find one in my area, does someone remember where is the list?

I too think a photocopy of your vaccination sheet is all you need to send because you have received all the shots needed for AOS. You don't have to finish the gardasil to apply, just start the treatment.

The DS3025 has worked for everybody who has done it that way that I've been keeping up with. There are some others whose whole medical package turned in at POE gets lost and they get RFEs because there is no medical record at all. There's nothing you can do to prevent that. It's their fault for losing the package, but they tell you to take all the blood tests and exam over again.

My vaccination sheet doesn't say DS 3025 even if it is the same table as below

The title is "vaccination documentation medical worksheet two' (i dont have the 1st one)

I got it at the medical and it says " for use in canada only"...

Can someone tell me if the form attached in my previous post is the DS 3025 and i can include this sheet?

I think that whatever they gave you will work fine, as long as it's been completed and shows you have all that's necessary. Don't worry if you don't finish all of the series shots, as long as you've started them. The I-693 which is the newest form I believe, has the same format as well, but you don't need it or anything else as long as the form you Do have shows you are complete. That is the key here.

That's the sheet I sent too. If you get an RFE, the only slowdown will be with the greencard and not the EAD/AP - at least that's what I noticed.
