I-485 question

Hi. I'm preparing to make a I-485 (Adjustment of status) package.

Please clarify me with the following questions

1. On the I-485 instruction, as an initial evidence #3, it says "Copy of passport page with nonimmigrant visa". It explains "If you have obtained a nonimmigrant visa(s) from a U.S. Embassy or consulate abroad within the last year, submit a photocpy (ies) of the page(s) of your passport containing the visa.

---I obtained my nonimmigrant visa (F-1) more than 3 years ago at a U.S. Embassy abroad. In my case, do I need to still submit, copies of pages of my passport without the visa page? Or I don't have to submit anything at all?

2. On the same I-485 instruction, as for the "Evidence of eligibility", will my case considered "#15 A Based on an immigration petition" or "#18 Other basis for eligibility"? I'm sending I-485 with I-130 and I-765 altogether. Do I still need to send an evidence of eligibility? If that's so, what can it be?

1) not harmed if you include it

2) A, as the I-130 is the immigration petition that provides you the eligibility. Without the I-130 you have no eligibility to adjust.
