one shot missing, need vaccination supplement

hi all, i need your thoughts on this

i didnt complete my vaccines back home, i still need to have one more shot

can i have a private doctor give me the shot or do i need to go to a registered civil surgeon for both the shot and to transfer my vaccination record to a form I-693A?

if i can go to a private doctor for that shot, what proof do i need to bring to the civil surgeon when s/he transfers my vaccination records to make sure all my shots are complete? all i have is my vaccination record from the medical exam i had as a requirement for K1, and it will show im missing the Td shot...

jeeleebee,

You should be able to get your missing vaccination from any doctor or clinic you chose. Then take the record of that shot along with your DS-3025 to a Civil Surgeon to get an I-693 Vaccination Supplement form filled out.
