Can anyone tell me of a civil surgeon in the Los Angeles area who will out the I-693 supplement for a nominal fee without asking for the hassle of getting actual vaccination records. I already have the vaccination supplement which was filled out by the panel doctor abraod and the information only needs to be transferred.
