Usually when you get #1, #2 follows.

Without the exact wording - it is typical that they are saying; "We can not verify your income (#2) without the tax return (#1) (and supporting document)"

I got a similar RFE. they claimed I wasn't above the 125% either. I'm sending them EVERYTHING now. past 12 months bank statements, IRS form 1040, W-2's, letters from my employers. everything I can think of. We'll see how that works out and I recomend you do the same.

As for the civil surgeon check the list. It wasn't too hard to get an appointment, however our required us to get the Gardisil shot series which was pretty pricey.

good luck

On #2, they WILL accept even just one cent over the 125%. The rule is absolute - one cent under and a co-sponsor would be required, dead on the line and you're okay. We were over it by only 35 bucks per annum and were fine without a co-sponsor :)

You don't have to get the whole series, just the first shot. And then you really don't have to go back (nor pay) for the rest.

3. I-693 REPORT OF MEDICAL

What kind of medical form did you send with AOS?

You are required to send I-693 with your AOS. Some people have been approved only sending in the DS-3025 Vaccination worksheet that the overseas doctor is supposed to give you when they do your exam. If you didn't send the I-693, that's what they now want.

Or it's possible they lost the whole medical exam results from overseas and want a new form filled out. This has happened to quite a few people. They sent in an I-693 with the vaccination section filled out but USCIS lost their exam and then asked for a new exam. This happened to my step daughters too.
