A quick question about Medical examination

I have a quick question about submitting a medical examination report along with I-485.

On the uscis instruction for I-485, it says,

"When required, submit a medical examination report on the form you have obtained from USCIS

A. Individuals applying for adjustment of status through a USCIS Service Center

B. Individuals applying for adjustment of status through a local USCIS office ".

When I read the uscis instruction for I-130, it says if I-130 is being filed concurrently with I-485, submit both forms to USCIS P.O. Box 805887 Chicago IL.

On the step-by-step guide on Visa Journey page, medical examination report was included in the checklist of I-485 package.

Now, this is making me confused. I believe P.O. Box Chicago is neither USCIS service center nor local USCIS office. And that makes it that I'm not requred to submit medical examination report for now. If this is the case, am I going to be asked to submit medical examination report later?

Or is it ok to include my medical examination report in the I-485 package and submit it to P.O. Box in Chicago?
