Asking about content of the I-693 report!

Hello, there! My husband and I went to the Civil Service Surgeon here in Allen park , MI., just this morning. I had my 2 vaccines(Hep.A, and Meningococall). I was just worrying now because the nurse assisted me just give me a sealed envelop of my vaccines.,labeled at the back Immigration documents, do not open. It is just a Vaccine History transferred from a written record, this is the page 3 of I-693. They said this is the only paper allowed to give me because I just had my vaccines only....In the instruction of the I-693, it was stated that I need the Part 1, and Part 5. We paid 200$ for the 2 shots. I was insisting to the nurse that the oher part needs to be included in the sealed envelop, but she said that`s the only thing they are doing for the vaccine shots for the immigrations!

Would this be okay?what if the Immigrations look for the other part of the Medical/Vaccination report? the vaccine report was already sealed and it is in my hands now. Please share us your insight for those who have been finished for their Interview.

I am sorry but I am not here to answer that but to ask/inquire if you already passed your I-485 or the application of change of status..meaning that you haven't receive any RFE yet am I right? You just done this just in case that they will gonna ask for the I-693?

Because I am in planning to submit the application this month but confuse if I will go to the civil surgeon for my additional vaccines & for them to transcribe my DS-3025 or just submit my DS-3025 and just wait for the RFE?

The sealed envelope they give you will be the only thing you submit to ur interview when asked.

I would advise for you to wait for your appointment letter because some are lucky not to have their interview and medical they just receive their green card in the mail.

I had my medical when i adjusted my status when i was sure i will have my interview and that was after i got the appointment letter.

It would be best for you to wait for your interview appointment letter before you will have your medical here coz there are some who are lucky that was not interviewed and no medical ,they just receive their green card in the mail. I had medical and interview when i adjusted my status but i did my medical after i got the appointment letter .

Hi, we got this done because this is one of the requirements in our AOS Interview on July21, in Detroit.. I don`t have any RFE, the vaccination report is the one I needed, and all my supporting documents indicated in the Appointment letter. ...Regarding your querries you should submit your Vaccination record(just a photocopy only) in your AOS Application so you won`t have any RFE.. The civil service surgeon office gave me a sealed envelope which i carry myself during my AOS interview and gave it to the CO. I am saying based on my experience as i can not give a comment am not sure about. I also submitted my I 693 together with my AOS application but no RFE .It may be different nowadays but like i said it is based on m y own experience.

My wife went to a Civil Surgeon in Livonia. She was given an AIDS test on the spot and we were told to go to the Oakland County Health Building. He gave us the address and the same day she got her MMR shot, HPV shot (if younger than 27), and TB skin test.

When he filled out the final I-693 he gave us a copy of everything that was in the envelope...

BUT, if you did not receive RFE and you have an interview,as per as listed in the interview notice, you have to bring the sealed envelope of I-693 to your interview. I think you should get everything done and documented on the I-693 and send as complete an application as possible the first time.

This information is not correct. She will NOT get a green card without getting her additional shots and submitting an I-693. She may get transferred to CSC, which means no interview, but she has to prove she has met all the requirements of immunization. The people that get a green card with only the DS3025 had every shot required in their country and all are documented on the DS3025. People who get more shots in the US must have them documented on the I-693 by a Civil Surgeon.

I-693 is a requirement for AOS. If you receive an RFE because you did not submit it together with other papers, then, you have to mail it to USCIS. BUT, if you did not receive RFE and you have an interview,as per as listed in the interview notice, you have to bring the sealed envelope of I-693 to your interview. I handed mine to the CO during my AOS interview. I was asked about it.

If you submit everything at once you "may" end up not even having an interview.

Hello..... I did not received any RFE . I just received my AOS Interview Appointment, it says there like "submit : A completed medical examination(I-693) and vaccination supplement in a sealed envelop(unless already submitted)" . During my POE I handed my Visa packet in the immigration officer and he gave me my x-ray result in CD, but during my vaccination in St.Luke, I was informed by the doctor that I need to get the additional vaccinations here in the U.S.since it was not available that time. So, I got my shots last Monday, and I was given a sealed envelop containing the completion of my vaccinations in one page of the I-693. Nurse told me that, the I-693 of my medical report in my POE was already submitted that`s why the only thing they need to do as Civil Surgeon was to complete my vaccines because they are not the one conducted my medical, unless they redo my medical they will issue me a completed copy of I-693.

Wait, why would you bring a NEW sealed envelop to the interview if you had already summited it with the original AOS package and did not receive an RFE? Correct me if i'm wrong but in the notice it says "unless already summited"

Hi, If you will remember we handed in our POE, our visa packet. As far as I know, all the information from us including our I-693(Report of medical from our home country) was included in that sealed envelop.That`s why officer returned our X-RAY in CD form during our POE. So, it means our medical report from Civil Surgeon in our home country was inside that visa packet(that report is the I-693). That`s why that packet was so important because it contains everything about our entry to USA.

I knew a friend of mine, she handed her Visa packet in her POE, but during her AOS Application. She was instructed to redo all her medical because her medical report(I-693)was lost. So she got RFE. It was not her fault because she handed everything in the immigrations during her POE, we don`t know how liable are they in handling and keeping our precious documents. She has to comply everything all over again just to get done all Immigration thing at all her expense.

In my part I have incomplete vaccinations in my home country during my medical, so what they did was they gave my DS3025 , when we file our AOS application I did not include I-693 because I have my DS3025, that`s in replacement of I-693. I showed that DS3025 in the Civil Surgeon, to find out which vaccines should I get, as per requirement for the AOS. When I had my vaccines last Monday here in our state I got 2 shots, they transcribe my new vaccine record to I-693 page 3, and put it in the sealed envelop, "noted: immigration legal documents, don`t open" and at the bottom of the note was the signature of the Civil Surgeon, inside the sealed envelop contains my completed vaccinations page. I was instructed by the nurse that I should hand it to USCIS officer during my AOS interview, and that`s soon.

So, wish us luck, I hope this help clear your confusions. That sealed envelop in our K-1 was our visa packet which we handed in our POE was different from my new sealed envelop now. My sealed envelop now was the report of my complete vaccinations transcribe to I-693.
