I have a suggestion to you..

If you are far from an National Statistics Office, I can recommend you this:

1. Call NSO Helpline Plus (02) 737-1111.

2. An operator will ask for details about your request like name, date and place of birth, LCR (local registry number) number, who would receive the document, address etc.

3. A reference number will be given to you.

4. Go to the nearest MetroBank and pay the fee of P300.

5. Soon as you paid the fees, NSO will immediately process your request.

6. Wait 7-10 days for the delivery of your requested document

This is what I do when I request for documents.

This has also been a question of mine coz it just so happened that my mom was the one who went to the NSO to get my CENOMAR, and her name appeared on the lower part of the page wherein it was stated that she was the one who requested, not me.

I think it will be fine cuz i know one of the member here in VJ did the same thing, she asked her brother to get a CENOMAR for her and she was approved during her interview.!!!Just dont get a CENOMAR in any travel agencies like what i did, there name appeared on the lower part and the consul ask me about it and i got the 221g to submit additional documents, which is a new CENOMAR...

Thank you for your reply. I already asked my mom to get me a new one and this time specify my name on the lower part (requesting party).
