AOS I-765 Office, Help need please!!!

Hi there, i am currently getting ready to send in all my forms for AOS and EAD which i will send at the same time.

My question is, on the I-765 form, after it asks your name, it says:

Which USCIS office? and a date?

I was wondering what i put in there? do i just put the chicago office where i`m sending this too, or do i have a local office i need to put in, or do i just leave it blank? Please, anyone that can help, i would love some feed back as i can`t get this stuff sent without it.

that means they ask you that if you apply work permit before your AOS application ? if yes than where you apply at your local uscis office? if you didnt apply before than you dont need to give answer

What you're looking at is a continuation (from left column) of question 11: "Have you ever before applied for employment authorization from USCIS?" - if you answered "NO" then leave the "Which office" "Dates" and "Results" sections blank.
