This is my IV packet checklist guys... kindly see if I forgot something? or If I shouldn't include something.

1. Bar Coded cover sheet

2. 4 Passport Size Photographs of the Beneficiary

3. Completed and signed form DS-230 Part 1 by the Beneficiary

4. Completed and unsigned form DS-230 Part 2 by the Beneficiary

5. Photocopy of the Passport’s Biographic Data Page of the Beneficiary

6. NSO Certificate of the Beneficiary

7. NSO Marriage Certificate of the Beneficiary

Also enclosed are photocopies of the following documents:

1. Passport’s Biographic Data Page of the Beneficiary

I don't see the original NBI certificate listed... You need the Original NBI Certificate included,,, as well,

AOS? the I-184? Also, is it a MUST to submit the 4 photos of the benficiary? I thought it was just 2 and the other 2 were for the medical exam stuff?

Also do you have to submit copies of the petitioner's passport and birth certificate? is this a requirement or something you just did?

HOW ABOUT I-1864 AFFIDAVIT OF SUPPORT?

My MIL will send the AOS also this Monday.

We'll both send in the same day the AOS and IV packet. The thing is the AOS was sent from the states. While I'll Fedex the IV from from here in the Philippines. The AOS and IV will just somehow meet at NVC. lol

Hopefully we wouldn't encounter any problem.

And the original birth certificate of the beneficiary and the original marriage certificate.

Here in the Philippines, the one that we submits esp BC and MC is the one that's coming from the National Statistics Office.

So the NSO ones on the list serves as Original.

I basically just provide everything I have here. My friend said, It would be better to be excessive than provide more later. So I included my husband's BC and a photocopy of his Passport's Biographic page. I don't think its gona hurt our packet, does it?

About the Photographs, I have seen few posts that they provide, 2 some 3, so I think to be safe, I'll provide 4, anyway, Photographs are so easy to be taken.

DS-230 requires only 2 photos, the medical requires 3. The DS-230 relates only to the beneficiary so the petitioner's passport and birth certificate are not required. Good luck :thumbs:
