K3 interview - documents regarding marriage and name change

Timeline

We have interview coming up for K3 in 2 weeks. My wife was previously married - got divorced - married me. Her middle and last name was changed from X (maiden) to Y (1st marriage) to X (post-divorce) to Z (marriage to me). She held passport in the name of Y, X and finally current passport in the name Z.

We did not notify or publish any name change in Gazette office. Is that a problem ? We assumed passport agencies are government offices so when we made new passports each time due to name change situation, they obviously know the fact and send this information to gazette as well. Not sure if they don't do that or do that. We want to know if we need to make a SELF-affidavit to capture the above fact that person X, Y, Z are all the same person.

My wife also has a child from her 1st marriage who is now going to be my Stepchild. His passport was made after mom's divorce so he has middle name of mother and last name of mother's maiden-name (X). Do we need to make an affidavit that says he is child of mother X/Y/Z who is married to ME.

Help us please.
