my second question was in the form I-485, the part two which lettter would my wife fall under there's an a-j option which letter should I choose??.

Medical exams are valid for one year. If she had the exam prior to the visa issuance and it is less than one year ago, she does not need a new medical exam.

for her medical, it is still valid for 1 year but she still has to submit form I-693..fill out the info part, visit a civil surgeon (accredited by USCIS ) for additional vaccinations and for him to sign the form..the surgeon will give you a sealed envelope of the signed I-693 form..

for I-485, its letter A...just attach a copy of the NOA2 of your I-130 application.
