I am filling my forms for Packet 3 in advance since I'll be without a printer starting next week (I might find one but I rather have the forms filled in advance anyway in case I have trouble borrowing one).

On question 30, DS-156 they ask if we have been issued a visa before. My problem is, when they ask WHERE do they mean WHERE the visa was issued as in what consulate gave it to us or WHERE did we go using that visa?

I'm pretty sure it means which consulate so this question might be stupid but when not 100% sure it's good to ask. :blush:

EDIT: Also on question 35, asking if we had a visa revoked or cancelled, does it count those previously issued and expired, that they stamp with the saying "Cancelled without prejudice"?

They mean the consulate that issued the visa. I'm 95% sure.

As for the edit... revoked or cancelled, to MY interpretation, means that it was taken away before its true expiration. I.e. if you were in the US, broke the law (or something) and got sent back to Brazil and they took away your visa, then you'd say yes. Otherwise, I think you say "no." That's just may take on it, though.

Yes it means which consulate it was issued at. I have a B2 visa it was never cancelled. 100% sure :D

Question 35 you answer NO :)

Good. Especially since I already did it like that and it´s a pain in the ####### to fill those forms over and over again. :P

Q.35 was a doubt to me ´cus my passport does say cancelled without prejudice, so I dunno if that was cancelled for them ´cus I believe when you do something bad it´s revoked, not cancelled, but it makes sense.
