I-131 AP - help with filling in form please!

I was originally on a J1 visa, got married to a USC in US and I have already sent off my i-485, i-130 and EAD. i am filling in my i-131, i asked the lady at INFOPASS how i could go home if required in an emergency as my grandmother is elderly and she recommended this and have a few questions:

Part 1. 3. Class of Admission - do I put AOS?

Part 7.1 Can I put more than one trip or do i have to put one trip?

As i do not know what the date of intended departure date or expected length of trip, what shall i put?

She told me to attach a copy of my I-485 NOA, and there is no fee - is that all i should include?

No. You put the class that you were when you entered the US. J1.

Part 7.1 Can I put more than one trip or do i have to put one trip?

Thats Upto you. Write More than One. You never know when you might need to make a trip/trips.

Just give an estimated date. Just to be on the safe side, the date should be kept atleast 2-3 months from date of application. I-131's take atleast three months to approve. your AP will be valid for 1 year from date of issue.

She told me to attach a copy of my I-485 NOA, and there is no fee - is that all i should include?

Attach a copy of your I-485 NOA to the AP application. Just to be safe, attach a copy of your passport and I-94
