DS-230 checklist for Mumbai

Want to doublecheck the list of documents to prepare for DS-230 for my Spouse's CR-1 Visa.

She will be dealing with the Mumbai Consulate in India. I just want to verify the following

is the correct list for the Mumbai Consulate:

DS-230 Part I - do we need to include part II? and if so do we fill it out also but not sign?

Her Passport - one photocopy (not notarized)

Her Birth Certificate - original and photocopy (not notarized)

Our Marriage Certificate - original and photocopy (not notarized)

Her Police Report - original and photocopy (not notarized) but NVC told me just to send photocopy?!

Her Photo - two visa photos

I will send the AOS I-864EZ ahead of time separately. Does the above all look okay?

Yes, you send in both. PART II DO NOT SIGN YET!

Her Passport - one photocopy (not notarized) - Okay no problem.

Her Birth Certificate - original and photocopy (not notarized) - Okay no problem.

Our Marriage Certificate - original and photocopy (not notarized) - Okay no problem. Is this in English.. If not, you need notarized translation.

Her Police Report - original and photocopy (not notarized) but NVC told me just to send photocopy?! -NO! Send 1 original and photocopy.

Her Photo - two visa photos - Send in 4 just in case they misplace one or it gets blown away... or some of such accidents. Put it in small baggie and stapple it to a bond paper.

DO NOT FORGET : Bar-Coded Cover sheet. and to stamp or put your case number on top of the documents.
