Form 1751 - Question 9 - another address

Timeline

Hi all,

In the Part III of the for, question n.9 (if you lived in any other address since you became a permanent resident) is mixed with questions such as "have you ever been arrested, deported, etc". Looks like a big deal so I don't want to screw it up.

I have moved a year after receiving my 2 year conditional card.

Now it is time to lift conditions and I assume I need to attach a sheet to the form and list my previous address (yes, I will also include the AR-11 with my application).

Any tips or templates for this?

Did I get it wrong by any chance?

I appreciate your advice.

We moved 3 times since I was granted conditional resident status and yes - I attached a sheet to the I-751, listing all addresses and dates we lived there. I also wrote on it that an AR-11 for myself has been filed online for every move on time (within 10 days of moving) and submitted copies of the NOA's we received for changing the sponsor's addresses with form I-865.
