.....when we file the I-485 Adjustment of Status, do we need to include the I-693 medical form signed by a civil surgeon too?

yes i do believe so. otherwise you might get an RFE from it... do you know the link where you can find a civil surgeon in your place? if not its listed on the USCIS website.

If your medical wasn't lost after POE you'd have to visit a civil surgeon with the copy of the vaccination sheet you got at your medical and ask him to transcribe that to form I-693A which is all you need to send as K1.

yes it is needed. But with mine, i did not include it with my application, I brought it with me during my interview. No RFE though.

The I693A is no longer used (since a little over 1 year ago). Part 5 of the new I693 is the vaccination supplement that can be done exactly as Laura described. You can do this if your visa medical is not over 1 year old when you apply for AOS (and its not lost). Otherwise, you will need a full medical.

If your medical wasn't lost after POE you'd have to visit a civil surgeon with the copy of the vaccination sheet you got at your medical and ask him to transcribe that to form I-693A which is all you need to send as K1.
