For the AOS package requirement for I-693A, a supplement for the I-693 Report of Medical Examination and Vaccination Record, can I submit instead the Vaccination Documentation Worksheet from the St. Lukes Hospital in the Philippines (DS-3025)?

some submitted the DS 3025 ang got approved without an RFE but some got an RFE requesting for the I-693.

I have no idea, only USCIS can answer that I guess. I have a read a post here I just can't find it now ,where one comment said that if your DS 3025 has a vaccination complete then you can submit it but if it is incomplete you have to see a civil surgeon for the I-693

We didn't submit the 693 either because we were running out of time and we procrastinated a bit, so we sent in the 3025, and never got an RFE. However, we read thru the posts and the chances are higher with getting an RFE it seems.

AOS

2009-01-12===> Sent AOS packet via UPS
2009-01-13===> AOS packet received
2009-01-28===> NOA's received in the mail
2009-02-01===> Biometric appt received in the mail
2009-02-06===> Completed biometric appt thru walk-in
2009-02-06===> Applied for expedited AP thru the phone
2009-02-14===> Received AP in the mail
2009-02-11===> Case transferred to CSC
2009-02-23===> EAD received
2009-05-02===> Green card received in the mail, no interview done.

Did your 3025 indicated a "Complete" or "Incomplete" vaccination?

I seriously have not found one person who got an RFE only because they used a DS3025 instead of an I-693 if all immunizations were complete. I've been reading posts for that information since last September. Medical RFEs are normally because they lose your medical file from you K1 exam or you were required to have a followup on a medical condition after arriving in the US.

The DS3025 will not say complete. Each shot will be marked as given or not age appropriate. If you had chicken pox then Varicella will say VH. If you have blanks, then maybe that's something yet to be done.

If any shots have to be given in the US, like HPV, then you must have everything put on an I-693. If you got everything done abroad and didn't have any follow-up requirements because of TB conditions, then the DS3025 will work.
