DS-230 & DS-157 questions

Hello All,

Our I-130 is moving forward and my husband just paid $400 for the IV fee bill online this morning (his evening). Now, I am completing my DS-230 and paused it for a while to ask for your advice regarding question #20 (list all places you have lived at least 6 months). Ok, when we applied for our marriage license in Manila, I used the address where I used to lived before but it was only five months. My question is, do I need to include that one on my DS-230 since it was the address in our marriage license? Will the consul ask about it if I would not include it on the address' list? I did not put it in any forms that we submitted but we are worried that CO might question this. In addition, about DS-157, previous employment...its asking you to provide the "name of your supervisor and the telephone", what if you dont remember her full name and contact number? It's maybe a shallow questions but I do need help.
