DS-230 questions.. again..

Timeline

I asked some questions yesterday on the DS-230 but was given the wrong information. Once I looked up the example someone linked to me from VJ, I noticed it was outdated. The current copy being located at http://travel.state.gov/pdf/DS-0230.pdf.

My questions again asked if anyone knows..

Questions 14 - 18 all reflect the spouse's information but my fiancee is a widow. Maybe the same general answer applies but I want to make sure.

14-18. Spouse's Maiden or Family Name, place of birth and marriage information.. Would this be her maiden name or current name or None/NA since she is a widow? I guess that same questions applies to all of the questions 14-18 including her marriage. Will they all be None/NA since she is a widow? Or should I answer any of them?

None.

It is a present tense question. You will notice on the DS156 the same question specifies to list the name of spouse EVEN if divorced, seraprated, etc. The DS-230 does not ask this.
