Hello! Can anyone tell me if the DS-3025 from the doctor is supposed to be one or two pages? The one my husband was given is only one page but at the bottom we found that is says 1 of 2 pages. Is this normal to only have one page?

Also, there is not much filled in on the form but at the bottom a box is checked that says "vaccine history complete for each vaccine, all requirements met"

Has anyone been told they need more vaccine's even when this box it checked?

FWIW, the K-1 has no vaccination requirement
