I only have a Vaccination Documentation Worksheet, DS-2015 and a sealed envelope which I believe contains the chest-x ray. What do I submit for K-1 AOS. The medical was done in October 2005.

Take the DS-2015 to a USCIS Approved Civil Surgeon and have them fill out a Vaccination Supplement (Supplemental Form to I-693) for you.

Phone a few Civil Surgeons in your area, Some may fill out the form for nothing. Be prepared for mis-information that you will need another medical - you do not!

Anyway, I needed an MMR jab when I got mine done. It cost me $60 for the shot and the form was filled out for free.

I only sent in a copy of the DS 3025 (the form is called Vaccinatin Documentation Worksheet) and we have our interview on Friday (No RFE.) So, I will find out if they still want any more documentation (from civil surgeons etc.) or not.

Since the biometrics are scheduled for after the AOS, I am not worried about delaying the greencard (its already delayed) - If they want the civil surgeon stuff, I will get it done around the same time (if not before) the biometrics. I will post about my experience on friday or saturday. Good Luck

I recommend you call around to several civil surgeons, you can get a wide range of fees. Good luck
