Parental Advice?

I hope this is the right place to file this lil post, Im sorry if I have misplaced it...

Ok Im going to PI in Aug/Sept to Marry my Fiance` and we are both excited. I found out I need to get the Affidavite due to the fact that the US government does not issue Capacity to Contract Marriage , so thats no problem I present my passport I pay the Fee im good.. But then I read this

Parent’s Consent or Advice. Under Philippine law, the legal age for marriage is 18. If the contracting parties are between the ages of 18 and 21, they must present written consent to the marriage from their father, mother or legal guardian. Any contracting party between the age of 21 and 25 must present written parental advice, i.e., a written indication that the parents are aware of the couple’s intent to marry.

Ok.. Well first off I was planning on Surprising my parents.. so thats now out the window (im 22) But what do I need to really provide? Just a letter typed out by my mother saying Yes I know my son is Marrying a Filipina and thats its? Then signed and dated? Is there an official form she needs to sign just to say that she knows Im getting married? If its just something she can write out/Type out and sign then ok, If its an official form where can I locate that at. I am going in Aug purely with the intent of getting married to the one I love, I dont want to mess it up and cause us to have to wait longer than we already have.
