Translating Birth Certificate Myself - Question

I'm translating the birth certificate myself. I'm attaching a seperate piece of paper with the following certification message as suggested by the USCIS:

"I________typed name, certify that I am fluent (conversant) in the English and ________

languages, and that the above/attached document is an accurate translation of the

document attached entitled__________."

My question is, what goes in the last blank space right after the word "entitled"? Is it the name of the form in my native language? Or should this be in English?

"Fluent/conversant" is an either-or question. As in you should either write "I am fluent in the English and..." or "I am conversant in the English and ..."

As to your question, I would write "Partida de Nacimiento (Birth Certificate)"--in other words, put it in your native language, then in parentheses in English.

In my country, Japan, documentation called "Tosekishouhon" is as same as birth certificate so that I fill in there that.

However, you can translate yourself, but you can ask your friend or translater who is native for your native language to fill in the certification message. My lawyer told me that I do not need certified translater.
