In my case, fiance is missing 1 shot --he will receive the shot end of this month-- so, i would need to submit a I693a supplement. Which i need to take his vaccination records to a local civil surgeon and then the civil surgeon will complete the form for us to mail in along with the I-485 ???

There isn't an I-693a unless you found an old copy somewhere on the internet. It has been replaced and shouldn't be used. Download the new I-693 and read the instructions. You'll fill in your personal information, then take the form to a Civil Surgeon (or mail to Dr. Arnold) along with whatever records you have of your shots. The Civil Surgeon will fill out the immunization section, sign the form, and seal it up for you to mail with your application. Call and ask the prices and confirm the CS will do only the immunization section for you. Many will charge too much or tell you that you have to take the whole medical over again. Shop around for the best price.

If your wife's medical is not more than 1 year old when you file, and all of the required vaccines are shown as complete on the DS3025, just send that in with the AOS. It has worked for many people.

You need that to show what vaccines he got at his medical. You should call and ask for a copy.

Yes you can. When we did it in October it was the transition time where people were filing AOS after that new form came out. The USCIS website said you must use the new I-693 and have a TB skin test, even if you had an x-ray for the K1 if your medical was after June 1. That's when people got all crazy about the transcribing of the exact same info onto a new form and worrying about how to get the TB test. We decided to take a chance and just see what would happen before we spent another dime. So it worked and no mention was made of a TB skin test. The USCIS has still not spelled it out in clear terms like you can send in the DS 3025 but from experience it worked for us. Here's some more people who have gotten recent green cards without an I-693--Dutti, Gary and Alla, Slightly Bonkers, Roselinda, ikarus, Talyss, HighwayStar.

Incidentally, when we were filing there were so many posts about the medical info being "lost". That was another reason we decided not to bother with the transcription. We figured why spend the time and money if they were going to lose the medical and we'd have to pay yet another civil surgeon to get that part straightened out.

Just make sure you have all the shots. I've heard of some panel physicians saying "you don't need any more shots" when they meant for the visa, because the shots don't have to be complete for the visa. So the immigrant thought they didn't need any more shots ever, only to find out that for AOS they didn't meet the requirements.
