Hi all,

my wife went to get her passport and they told her she needs to get form 137 from the school she attended that shows her grades from highschool. Why does she need to get that? It is not listed as a requiremnt on their website. Sounds crazy o me. Did anyone evr hear of this?

I guess she is from PI..well if she don't have any valid identification that is why she is required of this .It is not difficult to get this record from her school so give what dfa wants.

I agree with happywifeymom. DFA needs supporting documents that shows full name and birth details of applicant.

When I get my passport before I don't have valid ID'S.So the DFA request me to get a form 137 from school.Well they just want to know if what name she used when she was in school if its the same with the name she used all the papers she submit in DFA.That's not crazy..Yes it was not listed in the website but its one of the supporting documents to get a passport.

Why do you say it is not listed in their website?i wonder what this is for?and to think that's the latest website of dfa...
