Need suggestions and advice....my passport/visa lost. I am already here in US and the good thing is that I've got a photo copy for the necessary info on it...what shall I do....I am really worried coz somebody might use my identity using my personal info with my passport/visa...

call the nearest Phil embassy and apply for a new one...you might need to bring papers re: your AOS approval/greencard

B. REPLACEMENT OF LOST PASSPORT

1. Download the passport application form on this site and complete all the entries to avoid unnecessary delay. For minor applicants, a parent may accomplish the form on their behalf. Those who are either applying by mail or are not applying in person, the duly-accomplished application form must be notarized by a local Notary Public.

2. Submit three (3) identical passport-sized photos - 1.77" x 1.37" (3.5 x 4.5 cm) with royal blue background - taken within the last six (6) months. The photo must show the applicant's front view, clad in decent attire (not in sleeveless attire) and without eyeglasses. Digital photographs will not be accepted. Click here for more information on the photo requirements for machine-readable passports.

3. The applicant must also submit proof of his or her actual residence, e.g. driver's license, State ID, Employment ID, or other similar documents evidencing actual residence.

4. In order to clearly establish applicant's personal details, the original or authenticated copy of the Birth Certificate issued by the National Statistics Office (NSO) must be submitted in support of the application. NSO-certified civil registry records may be obtained through the following methods: (a) In Person at: the National Statistics Office, East Avenue (in front of SSS), Quezon City or at the Office of the Civil Registrar General, EDSA corner Times St.West Triangle, Quezon City 1104,Philippines; (B) By Postal Service System: address requests for certification to the Civil Registrar General, National Statistics Office, P.O. Box 779, Manila, Philippines; or By E-mail: send the details of the application to: L.Hufana@mail.census.gov.ph. In the alternative, the NSO also processes applications for certification online through its website www.e-census.com.ph. If the birth registry record of the applicant is lost or not available, the following documents must be submitted: a. Certificate of Non-Availability of Birth Record from the Local Civil Registrar of the applicant's place of birth. b. Original/certified true copy of the baptismal certificate, voter's ID or any other public document indicating birth particulars and Philippine citizenship. c. Joint Birth Affidavit by two disinterested persons concerning the applicant's birth particulars.

5. In addition to the documents referred to in item (4), the applicant must also submit other supporting documents which will establish his or her identity as a Filipino citizen, e.g. marriage certificate, school records, professional ID, or other similar documents.

6. To ascertain the fact of loss of the passport, an Affidavit of Loss of Passport, duly notarized and signed before a Consular Officer must be submitted. The applicant must state in the Affidavit the circumstances surrounding the loss of the passport, together with the details of the lost passport such as the passport number, date and place of issue, if known by the applicant. If the passport was stolen, a corresponding Police Report must be attached with the said Affidavit.

7. If available, a photocopy of the data page of the lost passport or other previously issued passport may be submitted. All those applying for the replacement of lost passports will be interviewed by a Consular Officer. Processing of Lost passport is subject to 15 working days waiting period from date of receipt of application. It is important to bring the original copies of the supporting documents required for a passport application to avoid delays in the processing of application. The original copies may need to be presented to the Consular Officer for verification purposes. These will be returned to the applicant after the verification process is completed. It is also important to bring additional photocopies of all the documentary requirements to ensure that sufficient copies are available should these be required.

You need to get a new passport. You have to do it ASAP since it would take sometimes for you to get a new one.

Are you done with your AOS? If not and if you are required to have an AOS interview, you need your I-94. You need to get a new one, just explain what happened.

I guess, you also need to report to USCIS that you lost your passport with your visa

Hi rheanick thank you for your time replying.... :) Yes I'm already done with my AOS but my status right now is still conditional so I have to remove it year after I was approved as the interviewer said.....Do I need my passport for removing of my status condition later?

I dont see passport on the list of required documents for the removing of status condition. You just need the passport for your international travel.

If you dont have any scheduled international travel, there is no rush. :)

Hi Guys......OH MY............You know what???............I tried searching my passport last night just hoping it was just inside somewhere around the corner.........Gladly found it.....I just overlooked it, I've searched it only on the folders of that box before many times but never found it....I found it inside bottom of my file box....it's hard to find because my box is black and unknowingly the case of my passport is black as well so if you will not make a hand on it you will never really saw it...Thanks alot Guys for your replies....I do really really really really appreciate it....
