So in my packet 3, they asked my "sponsor/s" to fill out a "SPONSOR'S FINANCIAL RESPONSIBILITY UNDER THE SOCIAL SECURITY ACT" attachment to the Affidavit of Support. They also attached a hard copy of it but no soft copy.

They told me I could scan it and my fiance can fax it back to me and they would accept it.

Anyways, I have scanned the text in and matched the formatting to the original form which you can use if you like to send to your sponsor/s. ;-)

Hope it helps!

They only asked for one copy of my birth certificate and passport, but I had a few copies just in case. The embassy does make copies too but at RM1 each, I think.

They also didn't asked me for the Social Security Act attachment. We had it all signed but they didn't ask for it. :S

What about vaccine requirements? I haven't gotten my Packet 4 but I got an pdf from the US Embassy in Malaysia listing vaccine requirements and there are SO many:

Mumps, measles, rubella, polio, tetanus, diphteria

toxoids, pertussis, influenzae B (HIB), Hepatitis B

varicella, pneumococcal, & Influenza

I only have records of having hepatitis shots and yellow fever shots...

What was your medical test experience like? Did you have to get many shots? Should I try getting some shots before hand. I don't want to have to have like 20 different shots all at once!

I don't keep any records of my vaccine and the doctor says since I'm 25 and I never have chicken pox before, I have to get MMR (Measles-Mumps-Rubella), Human Papillomavirus (I heard they no longer require this vaccine now) and Varicella. She also take my blood. The rest of vaccine I don't need it and the doctor tick "Not Age Appropriate" at my vaccination worksheet  My experience, it is not that painful because they shots at my ####### You will be fine....Don't worry

I can't recall the exact amount I pay but it is somewhere around RM500+ because Human Papillomavirus vaccine quite expensive. You have to call the clinic and make appointment with them and they will tell u how much u need to bring. The payment by cash :)

Thanks for the wish. I can't believe I'm a wife now :D

I paid only about $150 for mine because I had all my childhood and adult vaccination records. So no shots were needed.(Thanks mum!)

The panel doctor who saw me said that the HPV, varicella and pneumococcal shots are usually not needed. I don't remember all the details but it was a fairly laid back and pleasant experience. :)

How is everyone doing?

Quick question about the I-134 form. According to this form, the I-134 doesn't need to be notarized:

http://www.uscis.gov/files/form/i-134instr.pdf

But, in the P3 packet, more specifically on the checklist, it states "An original signed notarized Affidavit of Support (Form l-134) must be provided."

So, does it need to be notarized or not? For those who've been through the process, did you get yours notarized?

Do we have to make an interview for the medical appointment? I tried calling but no answer.

Is it just walk-in?

Ok, so I just did my medical today and paid RM223 (includes x-ray cost). Not too bad.

Got there early for my appointment at 10.30 and filled out the forms they had.

You need to bring:

~ Passport

~ Visa photo

~ Interview appointment letter

~ Vaccine records (if any)

~ They tell you to bring RM400 (credit card not accepted)

The Dr. was really pleasant. Only the MMR shot was required since I am over 18 and I already had chicken pox so no shot for that. They did a blood test, got a urine sample, and typical medical exam. I'll pick up my results in a weeks' time.

Then they sent me off with a map to a different place for the X-ray.

Getting there:

Drs ~ I took the LRT and it was walking distance from Masjid Jamek Stop. Bangunan Ming Annex. The receptionist gives good directions.

Xray - Star LRT Bandaraya stop, Wisma PKNS building - near Sogo (Daya X-ray Specialist Centre)

Was easy and I got done with everything before lunch. ;-)

Big thanks for the rather detailed info! :)

I still am wondering why there aren't many Malaysians active in the forums. Hmm. I mean, there can't be so few of us going through the K1/CR1/IR1 process... Thoughts?

I think not much Malaysia come to US through K1/CR1/IR1. Don't know why. I never meet any Malaysian in San Francisco and people always mistaken me as filipino. I miss good Malaysian food :hehe:
