"Ecuador Help" Airlines requesting form FS398

My fiancee is flying to the US via Copa airlines using a K1 visa and we noticed that they require her to hold a medical form FS398. Anybody knows what form is this? Is this the form found in the envelope that was given to her by the Consulate warning her not to open? I called Copa airlines and they had no idea what it was and told me to call the Consulate which costs $12 to buy a pin to call.

Warning:

- Those entering on a K-1 or K-2 visa must hold a medical form

FS398 and X-ray plates of chest (full size not necessary).

To facilitate handling upon arrival considerably, passengers

are advised to have these health documents ready in their

hands as they must have them at hand for the health control,

which takes place immediately after disembarkation.

Who is requiring this form? Where do these instructions come from?

I have searched and the only references I find to FS 398 are in old immigration court case files (in the 1960s) where they make reference to the medical form FS 398. It is not listed in any USCIS or DOS list of forms. It does not appear to be anything from the Department of Health, nor from the Foreign Service - the only FS designation that seems to fit. It is not from USDA.

I wonder if these are out of date instructions. The USCIS medical report, including the radiologists report on the x-rays, is contained within the sealed envelope. The applicant CANNOT open it - it must be opened by immigration. This instruction just doesn't make any sense and there doesn't seem to be any evidence anywhere I can find to get it to make sense. So, where do these instructions come from? Maybe you need to contact whoever is giving these instructions to find out more about form FS 398. good luck.

On the basis of Kathryn's research, and the airline's own confusion, and no massive threads in VJ fora about any such form being needed, I'd assume that all is currently well, and just perish any further thought. Plan that honeymoon, dude, si man!

If it is worth anything, I was not requested that form when I travel. I did with Continental. I have traveled with COPA and their web site is a mess. I think she should only show her visa and nothing else.
