The instructions say:

Based on admission as the K-1 fiancé(e) of a U. S. citizen and subsequent marriage to that citizen.

Attach a copy of the fiancé(e) petition approval notice, a copy of your marriage certificate, and your Form 1-94.

I take that to mean a photocopy so it is probably fine. I suppose they could ask to see an original if they wanted, but most likely won't.

To answer your questions about a certified copy...just go back to where you got the license and later registered the marriage (courthouse?) and they will provide you with a certified copy for a fee. It's probably just a photocopy that they stamp or emboss and sign a statement saying they certify that this is a true copy of the record. Ours is a reduced copy on nice paper with an embossing of the county seal and the statement/certification, but it could be done differently in every place that records marriages.

The answer depends of your state.

Virginia state for exemple, gives you a mc on the spot, which is not a legall/certified document, it is just for you and your husband. We sent in a request for two certified copies and got them three days later for the cost of $5.

Call the court where you got your marriage license from, or the marriage celebrant.
