I received I-864 in my packet 3, is I-864 the same as I-134? I also want to know how to / if 401K statement could be attached to any affidavit of support form. thanks

Generally the consulates use the I-134, but if they sent you the I-864 use that. They are both affidavits of support. If your income exceeds 125% of the poverty guidelines you do not need to include any asset information. Leave it blank.

401K would not likely be considered an asset anyway. To be an asset something must be capable of being liquidated within one year without harm to the family. Most 401k's do not allow liquidation without penalty, depending on your age of course. You could list it, and attach a statement, but there is no gaurantee it would be considered.

I also want to know how to / if 401K statement could be attached to any affidavit of support form. thanks If your income is not sufficient to meet the minimum and you plan to use your 401k's liquid value as a means to make up any difference, you should include the statement.

If, on the other hand, your income is sufficient, you really do not need to include either the value or the supporting evidence (the statement).
