Hey guys, Have a question for ya. My fiance has her crba interview coming up on June 16th. I want to make sure that she has absolutely everything that she will need. I have already sent her the DS-3053 form: Statement of Consent for Issuance of U.S passport and the DS-2029. Had them both notorized of course. Sent my birth certificate, military discharge papers DD214, passport, divorce decree etc...I have read the Legitimation Requirements in detail. I guess my question is in regard to evidence of parentage. It states, If the applicant was born out of wedlock to a U.S citizen father, the father must submit an affidavit containing the following information:

A) Acknowledgement of paternity of the child

B) Agreement to provide financial support to the child until child reaches the age of 18, and the father's whereabouts/location, during the approximate time of conception of the child.

The second part is easy to confirm. I have my passport with entry/exit stamps. Ok..finally my question. Has anybody submitted the affidavit? Do I just type up a letter acknowledging paternity of our child and agreeing to support her until she reaches 18 then have it notorized or is their a specific form for this. If so, I sure can't find it. Anyone that has done this I would really appreciate your input and advice on how to proceed with this so I can get it completed. Thanks so much for your help.

Hi Mike! Yes! You have to do a letter of affidavit of parentage indicating that you are ---

1) acknowledging the child;

2) agree to provide a financial support until she reaches 18.

3) Also you have to indicate where you are specifically the location at the time of your child's conception.

All these three should be seen at your affidavit of parentage and it MUST be notarized.

Thank you very much for your help. Did your fiance just make an affidavit on the computer and have it notorized?I actually did find a downloadable form for Affidavit of Parentage. Found it on google. The form is from the U.S embassy. Here is the link for it below. I know that you don't need it but it may help someone else. Thanks again.

Welcome. My husband did computerized affidavit of parentage and notarized in the US. Actually, he also found that form and he filled it out but the prescreener in the ACS rejected it. I don't know why. That's why he made another notarized affidavit of parentage plus the completed form of DS 2029. But I don't know with your case. Maybe you could try that also it might help you guys. Goodluck!

Welcome. We just submitted last week our additional documents via courier. Actually, another set of documents. And now, we're waiting a call from ACS. Hopefully everything's okay and hopefully they approve it.

IF you don't here from ACS pretty soon call them. That's what I had my fiance do after we had waited a while for them to contact us and when she called they already had the documents and scheduled her crba interview. If she would'nt have called them we might still be waiting. I really hope everything goes ok for you guys. I am sure it will. We will be thinking of you. Good luck.
