My husband is here on a K-3 we are about to send in his AOS. I keep reading that you have to send in the I-693 but, I do not have this. All my husband was given was a DS-3026 medical exam worksheet from the civil surgeon and, a DS-3025 vaccination worksheet.

Where can I get this I-693 and do I need it. Is was we have sufficiant?

Received NOA1: October 3, 2007

Sent I-129F: October 4, 2007

I-129F NOA2: April 3, 2008

NVC Received: April 22, 2008

Left NVC: May 15, 2008

Interview: June 18, 2008

U.S. Entry: August 11, 2008

AOS
