many people have adjusted successfully with only submitting the DS-3025 when the visa medical report is less than one year old

YMMV

and if all the immunizations are complete without further shots needed in the US. That's exactly what we did and many others recently approved did so too.

Submit the vaccination supplement he got at the medical exam, it was enough for us.

The medical exam from the visa is only valid for 1 year so you should consider that, too. I filed my husband's AOS packet in March and figured we'd be ok because his medical from the K1 was valid until April 11th. They say in the instructions that "if you're filing within one year of the medical exam then you need only submit the vaccination supplement." They don't say anything about how long it will take them to get to your case and if the medical exam expires in the meantime, will you have to do it again?

On Friday I got an RFE notice in my email and although I haven't gotten the detailed letter in the mail yet, I'm 99% sure it's because #1 the medical expired by the time they got to our case and #2 I was absolutely ####### about sending them everything else they needed. Patiently  awaiting the RFE letter.

Technically, everybody is required to submit an I-693. If you have a valid medical, you do not need parts 2-medical, 3, and 4 filled out. All you need is for a CS to fill ourt part 2-vaccinations and sign in part 5. That's it. If you don't have a valid medical, you need all the parts of the form filled out.

You can try and submit the DS-3025 instead of I-693. If immigration didn't lose your medical results and if your vaccinations are complete, you might be able to get away with it.

But you may get an RFE, and you will then have to do a FULL exam again. That's what the RFE will most likely say, "to submit a completed I-693". You will get delayed by about 2 weeks if you get an RFE.

The RFE may delay your EAD/AP. In our case, my stepdaughters received their EAF's the same day as the RFE for medical. We just sent the DS-3025 (all vaccinations completed), but they said their file didn't include a medical, so they had us do it over.
