Okie I dont really know my father, my mother divorced him like 25 years ago and he hit by a drunk driver about 17 years ago. I do know his name but I dont know anything else about him so I just want to know what should I put on the G 325A form.

I suppose you will need to supply more info in order to get an educated response.

On the G-325A, where it says "Date, City and Country of Birth (If Known)" - if you don't know then put "unknown". Check all your personal documents first, like birth certificate, and make sure the city where he was born isn't listed on one of them. Where it says "City and Country of Residence", put "deceased".

08/18/2010 - AOS Interview - APPROVED!

05/01/2013 - Removal of Conditions - APPROVED!

You will notice the G-325a asks for parent information "if known"

The correct answer for you is "Unknown" We did the same for my wife...no problems. If he is deceased from this accident then the correct answers are "deceased"

This is 100% correct. This is exactly what we did, and had no issues.
