Visit www.uscis.gov and click the "Immigration Forms" tab at the top of the page. Scroll down until you see I-693, and click on it. Choose "download form I-693" from the next screen.

Having said that of course, if you're going to make a personal visit to your local civil surgeon's office, they probably already have the forms there.

Keep in mind is you are adjusting from a K-Visa, and your medical was less than a year ago, then ONLY the VACCINATION part of I-693 needs to be done.

Many doctors like to insist on a full medical, you need to show them the instructions for form I-693, and INSIST that they do their job and do just the vaccinations.
