Where do I get I-693(A)?

One of the documents needed for AOS is the I-693A as described below (from VJ Guide in this link: http://www.visajourney.com/forums/index.ph...;page=k1k3aos)..

"I-693, Medical Examination of Aliens Seeking Adjustment of Status. (If you already have this from your overseas Visa Interview submit a copy that medical check form; No fee is required in this case. If you are a K-1 fiancé(e) or K-2 dependent who had a medical examination within the past year as required for the nonimmigrant fiancé(e) visa, you only need to submit a vaccination supplement (I-693A), not the entire medical report. You may include the vaccination supplement with your adjustment of status application.)"

Is this the same as the Vaccination Documentation Worksheet given to me by St. Lukes? It's the only medical document left with me now after I submitted my entire medical report to the Public Health here in Phoenix.

Hoping someone can help. Thanks so much!

I-693 is a medical exam done with an authorized USCIS civil surgeon here.My medical at St.Lukes was still valid when i got here 2 years ago but i had another medical when i adjusted my status.

Hi. Did u get your I-693 while assembling the AOS package and sent it with that package? Or did u have to do that while the AOS is being processed? Thanks so much for the quick reply. :)

I brought my I-693 during my AOS interview.No i did submit my AOs application first then brought with me my I-693 when i had my interview.Some are lucky not being interviewed ..some are even luckier was not asked for this I-693..in my case i was interviewed and asked for my I-693..it was not mentioned in my appoitnment letter but i did it anyway just in case and buti na lng kasi hinanap nang nag interview sa akin at meron akong ready na I-693 so approve agad.
