Hello VJ members; i just received a letter from USCIS

The Letter Says:

Request for Additional Evidence (I-485)

There has been a vaccination history submitted with the I-485,but there is no evidence of a medical examination.

Please submit a completed I-693,making sure to complete each box.

NEW MEDICAL FORM I-693: pls. submit properly completed and signed from I-693, report of Medical Examination

and Vaccination Record. and so on and so fort.....

Tuberculin (TB) Skin Test: Current regulations now require that the tuberculin skin test and/or chest X-ray

be completed by all aplicants 2yr of age and older. Your form I-693 reflects that you have not had a tuberculin skin

test or chest x-ray performed. A chest xray is required only when the reaction to the skin test is 5 milimeters or more.

and so on and so fort....

Tuberculin skin test: Your skin test shows a reaction greater than 5 milimeters and you did not have a

chest xray report completed. As such, you must complete a chest xray or provide other evidence to establish

that you have had follow up treatment for tuberculosis and that the condition no longer exists. iF a chest xray

is required, you must also include a copy of the xray report report,whether a copy or an original,signed by the

radiologist and on official hospital or medical office letterhead will be accepted.

You need to have a medical exam by a civil surgeon. Visit the USCIS web site for a list of approved civil surgeons in your area. They will complete the I-693 for you. Quick painless & you go back in two days for the documents ( they have to read the TB test ) & that's it. The civil surgeon will use a small needle to inject some harmless testing fluid (called “tuberculin”) under the skin on your arm and you will be ask to be back after 2 days (approx. 48-72 hours) since you had the skin test. Your skin will have a red bump on the area where civil surgeon injected. They will measure the size of the bump and if it is greater than 5 millimeters that means you have a positive TB skin test but it doesn't mean that you have Tuberculosis. Positive results may be due to a latent or active TB infection, or occasionally due to a false positive. If that will be the case, you will have to take x-ray. Only accredited civil surgeon can give you the I-693.

I think the RFE means they lost your medical information from St. Lukes. It would have been in the brown envelope you turned in at POE, but that did file didn't get matched up with your AOS application file. It's saying that you are OK on immunizations, but they don't have records of your blood tests or a chest x-ray (or TB skin test instead of x-ray). Maybe if you posted with a title of "Lost Medical" you would get some answers of people who had the same experience. Most just go have their medical done all over again at a Civil Surgeon. It seems to happen alot. Their mistake, but will cost you more money to resolve it.

I think the RFE means they lost your medical information from St. Lukes. It would have been in the brown envelope you turned in at POE, but that did file didn't get matched up with your AOS application file. It's saying that you are OK on immunizations, but they don't have records of your blood tests or a chest x-ray (or TB skin test instead of x-ray). Maybe if you posted with a title of "Lost Medical" you would get some answers of people who had the same experience. Most just go have their medical done all over again at a Civil Surgeon. It seems to happen alot. Their mistake, but will cost you more money to resolve it.

I didn't sent I-693 for our AOS application only vaccination records from saint lukes.

orginal,signed by the radiologist and on oficial hospital or medical office letterhead will be accepted.

You need to have a medical exam (another one at your expense) and make sure it includes all that they have asked for and send them the results.

If it is less than 1 year since you had your St. Lukes medical you DON'T need full medical. You only need a USCIS designated civil surgeon to transfer the information from the DS 3025 vaccination records St. Lukes gave you to the DS 693.

You can normally get the TB test at your local health department for free . or a small fee.

Take the record of that to the civil surgeion also.

Dr Arnold in Texas does the transfer for $30 by phone and fax.

If it's been over a year since you had the St. Lukes you'll have to have a full medical from from a civil surgeon in your area.

K1 denied, K3/K4, CR-1/CR-2, AOS, ROC, Adoption, US citizenship and dual citizenship

No, you didn't understand my post. If you had a medical in your country, you don't have to send a medical report. It's in your immigration files. St. Lukes sends your medical information to the Embassy (or you take it. Not sure how it goes in Phils.) So when you interviewed for your visa, your medical informations is reviewed. It's in your file. You get your file in a brown envelope to present when you enter the US. Your medical info of your blood tests and chest x-ray went from St. Lukes, to the Embassy, to the US airport where you turned it in. When you apply for AOS, your K1 file is supposed to be sent to put in your AOS file. That's how they know your medical results. They sent you an RFE because they don't have the blood test or chest x-ray reports that should have been forwarded by USCIS from the airport. They said your immunization is fine (because that's what you sent.) BUT they (USCIS) has lost your medical report.

If they hadn't lost that report, then you wouldn't need a TB test. It says

Current regulations now require that the tuberculin skin test and/or chest X-ray

You had a chest x-ray in Phils, but they've lost it.

You also had these blood test in the Phils, so if they don't have it, then it means USCIS lost it or never sent it.

You are most likely going to have to go take another medical exam similar to the one you had at St. Lukes, only because somebody in USCIS didn't do their job and send the one from St. Lukes. It wasn't your fault. You did okay, but it will cost you money because of their mess up.

In the US it is a standard health practice to test for TB by placing a PPD skin test as described above. The fluid that is injected is not actually testing for TB but for ANTIBODIES to TB. But antibodies to TB may have been aquired not just by infection but also by vaccination against TB. Many countries routinely vaccinate their children against TB. If you were ever vaccinated you need a chest Xray instead of a PPD to screen for TB because you will always have a positive PPD reading. I told the nurse at our medical exam that my husband was vaccinated as a child and that the PPD was both a waste of time and money, he needed only a chest X-Ray. But USCIS has their rules and there is no allowance for deviation. A PPD MUST be done whether it is indicated medically or not. So we had to waste the money and time on the PPD which was positive (Gee! What a surprise!) and follow it up with the $45 X-Ray which was negative.

form I-693 reflects that you have not had the serologic test for syphilis and HIV performed. Pls contact civil surgeon

You also had these blood test in the Phils, so if they don't have it, then it means USCIS lost it or never sent it.

You are most likely going to have to go take another medical exam similar to the one you had at St. Lukes, only because somebody in USCIS didn't do their job and send the one from St. Lukes. It wasn't your fault. You did okay, but it will cost you money because of their mess up.

As what i understand about this letter is that they just found out that my result is greater 5mm because

our medical result and embassy file was forwarded to them during our Port of Entry or (POE)

Thats why it makes me confuse because it's in the letter

during my medical exam i took chest xray and thats it

my medical result is no problem according to them how come they put this on the letter

please help...please correct me if im wrong....

****Tuberculin skin test: Your skin test shows a reaction greater than 5 milimeters and you did not have a

chest xray report completed. As such, you must complete a chest xray or provide other evidence to establish

that you have had follow up treatment for tuberculosis and that the condition no longer exists. iF a chest xray

is required, you must also include a copy of the xray report report,whether a copy or an original,

signed by the radiologist and on official hospital or medical office letterhead will be accepted.****
