I received Request for Evidence (RFE) today for a Report of medical examination and vaccination record form (I-693). I did not know that it was notified my by the examining physician in my DS-3026 that I was required to have a medical follow up within six (6) months of my arrival here in US for my thyroid condition. I only submitted the DS-3026 form in my application in lieu of I-693. Now, my concern is that I have been here in US for more than 6 months. I've been here almost 9 months already since I arrived in America.

Will my medical examination and vaccination record form (I-693) valid when I will comply it? It's long overdue now. I am required to submit it until June 12, 2009.

How long it takes to complete the medical examination and vaccination record by the civil surgeon approved by USCIS?

Any ideas of my concern...please do share...I am so upset of this.

We did not understand about the medical exam & got an RFE on a Thursday, found a civil surgeon 15 minutes away from where we live (this office only did walk-ins NO appointments) went on that Friday, got his vaccinations, TB test, HIV/Syphellius test & we were in & out in about 20 minutes. We had to go back the following Tuesday to have the TB test read. We went in they checked the TB test, my husband signed one document, they gave us a sealed envelope for USCIS, & a copy for our records & that appt was less than 10 minutes. Immediately put in the mail, & he got his EAD in the mail today (2 weeks from the day they received the medical exam)

Be sure to send the sealed envelope to them with the original RFE (make a copy for your records) & put the sealed exam envelope inside another envelope they were very specific to NOT OPEN & NOT WRITE ON THAT ENVELOPE!

05/09/2009 Received EAD card in mail YAAAAAY

05/12/2009 Applied for Social Security Card

05/15/2009 NOA received with I-485 interview date 07/20/2009

07/21/2009 Email received " Card Production Ordered " YAAAAY 139 days!

07/24/2009 Welcome letter received! & Approval of I-130 also received! Yaaay

Thanks kittykisses73 for that reply..that's really a relief on me. I still have few more clarifications...So, I need to fill out the part of I-693 forms that concern my personal info before I will go to the physician and then the rest of it will be taken cared of the physician? Am I right?

Actually, they attached an envelope with address of USCIS of it. Is this the envelope that the physician will use for the I-693 forms?

That is probably for you to send it back in, the Dr will give you one sealed that you will put in the envelope you received from immigration. Ours came in a large brown envelope that was like one you would get Xrays in maybe. . . But if you have to put them in something larger than they provided I would use the envelope they sent you like a label & just attach it with tape maybe :o).

Also you only fill out the first portion about whoever is getting the exam BUT DO NOT SIGN IT! The civil surgeon will advise you when to sign because it has to be done in front of them. Make sure also you take the ORIGINAL RFE & your passport with you (Also make sure you understand the preffered method of payment because the office we went to only accepted cash!)
