I'm going to Bumrungrad hospital in Bangkok for my med exam. My interview is slated for early in June. I saw a medical exam form on the embassy web site in pdf format. Is that the one we should use as reference when we go take our exam? Does that list all the required vaccines?

Here is the sample pdf:

I called NVC today and they said our package left there a few days ago. They said we won't get any letter from the US embassy other than what was already emailed to us by NVC.

The hospital knows exactly what needs to be done in every aspect. They do an amazing job so no need to worry.
